* for prepaid electricity voucher users, attach a photocopy of the most recent 1-month electricity payment receipt from PLN
-1 photocopy of KPS / KKS (if applicable)
-2 photocopies of prospective student's ID Card (KTP)
-2 photocopies each of Father's, Mother's / Guardian's ID Card (KTP)
-Health Certificate (Original)
-Non-Color Blind Certificate (Original)
-Drug-Free Certificate (Original)
-Color photographs of the prospective student: 2×3, 3 x 4, and 4 x 6 cm, 2 copies each (blue background, wearing white school uniform)
-3R size photographs of the house: Front View, Living Room, and Kitchen (use photo paper)
Registration documents must be placed into a folder with the designated color according to the accepted study program:
-Machine Maintenance: Blue
-Mining Technology: Orange
-Plantation Product Technology: Dark green
-Agroindustry: Purple
-Plantation Crop Production Technology: Light green
-Road and Bridge Construction Engineering Technology: Brown
-Electrical Technology: Dark red
-Information Technology:Grey
Additional Documents: (deadline for additional documents is 25 May 2018)
– Show payment receipt (according to UKT (single tuition fee)) for registration fee + 1st semester tuition
– Present Registration Card (PMDK-PN Registration)
– SKCK (Police Record Certificate, original) = 1 sheet
– Legalized photocopy of Diploma / grades = 1 sheet
– 6000 duty stamp = 2 sheets
– Recent passport photos size 3×4 = 2 sheets, and 2×3 = 2 sheets
– For males = White shirt + Black tie (blue background)
– For females = White shirt + Black tie (red background), black headscarf (for those who wear a hijab)
