2020 Yudisium Registration

Students are hereby informed that the Yudisium registration period is now open online via the Academic Information System (Siakad) from 7 – 19 September 2020. The following are the guidelines for online Yudisium registration.

1. Download the yudisium form HERE and complete it in full.
2. Scan/photograph all yudisium registration documents along with other required attachments, namely:

  • Scan/photo of the fully completed yudisium form
  • Scan/photo of the administrative clearance form (UKT (single tuition fee), laboratory/workshop, and library) DOWNLOAD HERE
  • Scan/photo of the Final Project examination official report (BAP)
  • Scan/photo of the final project revision approval sheet – DOWNLOAD HERE
  • Scan/photo of the Final Project approval sheet
  • Scan/photo of the plagiarism validation sheet – DOWNLOAD HERE
  • Scan/photo of the OP2LK certificate
  • Scan/photo of high school/vocational school (SMA/SMK) diploma.

3. Upload all scanned yudisium registration documents via your respective SIAKAD account under the menu Perkuliahan-Data Yudisium-Yudisium mahasiswa.

4. Students are also requested to update their data in SiAKAD via the website siakad.politap.ac.id (not via the Android app). Complete all personal data, including General Information, Domicile, Parents, Guardians (if applicable), and School. Please double-check your NAME, PLACE AND DATE OF BIRTH, and NIK, as this information will appear on the diploma.

5. Once all documents have been uploaded, please confirm via WhatsApp to:

6. Registration for the diploma photo session can be completed during the same period, from 7 – 19 September 2020.

  • Photo session registration is conducted directly in the BAAK room through Bpk. Effendi
  • Wearing a mask is mandatory when registering
